Participation Consent

I confirm that I am the parent/legal guardian of the above-named child and give permission for my child to participate in the fitness class/membership organised by the gym named above. I understand that the activities may include age-appropriate fitness exercises, movement-based activities, and group participation under supervision. * I confirm

Applicable To All Under 16 Memberships

My child is physically fit to participate in these activities.
I have informed the gym of any medical conditions, allergies, inhalers, injuries, behavioural needs, or additional support requirements that may affect my child’s participation.
I give consent for appropriate trained staff at Go:ver Gym to give first aid on the above-named child if needed.

Applicable To 'Hyrox Next Gen' Memberships:

If the child is partaking in the NXT GEN classes, they are at the appropriate age for the class.

Applicable To 'Grass Roots' Memberships:

If the child is joining the grass roots membership they are within years 9, 10 or 11 at school or the equivalent age.
Barbell use is not permitted on the Grass Roots membership unless the Grass Roots Barbell Membership has first been completed and the child has been signed off on barbell exercises by an instructor.

Applicable To 'Train With Your Kid' Memberships:

If the child is partaking in ‘train with your kid’ they are 10-15 years old and will be supervised at all times by a parent/guardian.

Gym Etiquette and Safety

I confirm that I have read and acknowledged the following prerequisites of being a valued member of Go:ver Gym:

On arrival sign in on the tablet at reception.
No outdoor shoes in the gym.
To take a purple cloth and spray from reception & wipe down your equipment after use.
When finished with a workout/equipment, please ensure you’ve put all that you have used away & in the correct places.
Train hard and be kind!
Please use the correct bins for waste and recycling and put chewing gym in the bin.

Photography & Video Consent

I understand that photographs and video footage may be taken for promotional and marketing purposes. By ticking the box I give permission for my child’s:

Usage & Storage of Media

By ticking the box I acknowledge and agree that:

The gym may edit, reproduce, publish, or distribute the recorded material for promotional purposes.
No payment or compensation will be provided for the use of my child’s image or likeness.
The gym will use images and footage appropriately and respectfully.
The gym will not knowingly publish full names alongside images of children without additional consent.

Withdrawal of Consent

I understand that I may withdraw consent for future use of my child’s image or recordings at any time by contacting the gym in writing via email to reception@govergym.co.uk I understand that this withdrawal will not apply to materials already published or distributed before the withdrawal request was received.

Waiver of Liability

I give my full consent for my child to participate in classes and/or memberships organized by Go:ver Training Ltd and held at Go:ver Gym. I am aware that although all steps are taken to ensure participants safety, as with all physical activity there are inherent risks which can include accidents, falls, injury or even death. I confirm that I know of no medical, physical or mental reasons why the mentioned child would not be capable of performing the physical activity in which they wish to participate at Go:ver Gym. I acknowledge my responsibility in communicating to a staff member prior to commencing activity, any physical and/or mental concerns affecting the mentioned child which could conflict with use of the facilities at Go:ver Gym. I assume all risks of injuries associated with use of Go:ver Gym and its facilities including, but not limited to falls, contact with other participants, exercises and techniques taught to my child by trainers employed by or providing services for Go:ver Training Ltd. and all other risks being known and appreciated by my child. Having read this waiver and knowing these facts, in consideration of accepting the mentioned child’s use of the gym facilities and of services provided to the mentioned child at Go:ver Gym by Go:ver Training Ltd. I agree to Hold Harmless, Waive and Release Go:ver Gym, its directors, employees, representatives, anyone else providing a service at Go:ver Gym and successors from any responsibility, liabilities, demands or claims of any kind arising out of the mentioned child’s use of facilities at Go:ver Gym or out of their participation in training, classes or anything else organised by or on behalf of Go:ver Gym.

For ‘Hyrox Next Gen’ Memberships Only

Upon signing your child up to their first block of ‘Hyrox Next Gen’ sessions, they will be given a T Shirt which we encourage our Hyrox Next Gen team to wear to each session.
